Out-of-network Disclaimer

Unlike many insurance-based clinics—where patients often spend limited time with the physical therapist and much of their session with non-licensed support staff—your care here is provided exclusively by your physical therapist from start to finish. This model allows for focused attention, personalized care, and a higher standard of clinical excellence to help you move with confidence and return to the activities you love.

Physique operates on a cash-pay, out-of-network model, which allows you to receive one-on-one care exclusively with your licensed physical therapist from start to finish. As a result, patients are responsible for payment at the time of service.

For those who wish to seek reimbursement through their insurance, we offer super billing. A super bill is a detailed receipt that you can submit to your insurance company for potential out-of-network reimbursement. It includes codes, descriptions, and charges from your visit but must be submitted by you directly to your insurance provider. It is the patient’s responsibility to verify whether their plan covers out-of-network services and what portion, if any, may be reimbursed.

A packet with guidance about super billing is available upon booking your appointment, and we can review any questions you may have together during your initial evaluation. While Physique provides support and guidance, please note that we do not submit claims to insurance on your behalf. This approach allows you the flexibility and transparency to manage your care while receiving focused, personalized therapy from start to finish.

FSA/HSA Disclaimer

Physique accepts FSA and HSA cards for eligible services. Please confirm with your plan administrator that physical therapy qualifies under your specific plan.
